Most states provide free or low-cost mammograms for uninsured and underinsured women through initiatives such as the National Breast and Cervical Cancer Early Detection Program (NBCCEDP). This program operates across all 50 states, the District of Columbia, and US territories, offering screening services to low-income, uninsured, and underinsured women.

Each state administers its own program, typically through its Department of Health, which coordinates services and assists eligible women in enrolling to receive complimentary or affordable screenings and diagnostic evaluations. Additionally, women diagnosed with breast cancer through these programs may be referred to Medicaid for treatment.
