Mammograms After Sunday/Spiritual Service

MASS, created by The Beautiful Gate Inc., is the signature breast health education and screening program of the Florida Breast Cancer Foundation. The MASS program’s goal is to reduce late-stage breast cancer diagnoses in uninsured women in the State of Florida by providing breast health education and access to *FREE mammograms provided by the Florida Department of Health Mary Brogan Breast & Cervical Cancer Early Detection Program.

To attend one of our Breast Health Education Presentations, please fill in the required information and then answer all the questions below.

Please note, according to the Affordable Care Act, if you are insured most private health insurers must provide coverage of women’s preventive health care such as mammograms, screenings for cervical cancer, prenatal care and other services with no cost.

In order to be screened on the mobile unit you must be able to walk up and down steps with a handrail for assistance. In the last six weeks, have you had a COVID vaccine or do you plan to have one before your appointment?
